Pilonidal Disease/Sinus (PNS)
This is a chronic skin condition mostly affecting males between puberty and 40 yrs of age. The disease manifests as recurrent infection or pus discharge from sites in between the buttock cleft.It usually occurs in young men with increased hair on the back or between the buttocks.
Is PNS a serious condition affecting the underlying bone
No. PNS is not a serious or life threatening condition. It is more like an irritant due to its
recurrent nature and affects social life and hygeine in that area. The infection is usually limited
to the skin, and does not go down to the bone.
Can female be affected by this diseases
Yes very often Pilonidal disease occurs in females who are hairless in the back or between the
buttocks
What is the causes of Pilonidal diseases
The exact cause is not known. Initially it was thought to be a congenital condition, but now it is
recognized as an acquired disease with pioneering work done by a Greek physician Dr George
Karydakis.It is caused by shed hair collecting in between the buttocks. This hair then enters the
skin in that area, and causes inflammation and infection.
What are the predisposing factors
Some predisposing factors are obesity, deep cleft between buttocks, excess hair in the area
between buttocks or even excess back hair, sitting for long
Can Pilonidal disease be cured
Yes, with a well chosen procedure it can be cured. The recurrence rate has decreased from 20-
30% to less than 2%
What are the various surgical techniques for this kind midline procedures
Here the surgical scars remain in the midline
1 Wide excision : wound is left open and heals gradually in over more than a month .The
resultant scar is in the midline between the buttocks
2 Pit picking : This is an OPD procedure with excision of just the sites of the pits. Againg the
wounds heal gradually and small scars are in midline.
Off midline procedures
1. Karydakis procedure : The whole area of the pilonidal disease is excised enbloc and then the
defect reconstructed placing the scar to one side off the midline. It thus removes or flattens the
cleft between the buttocks which is considered as a key factor to cause the disease.

What procedures do you recommend?
I usually do the karydakis procedure, but offer individualized treatment ranging from :
Incision drainage abscesses
Simple pit picking
Karydakis procedure
Along with whatever procedure I perform I advice
Decrease weight
Remove all back body hair if present. Best is LASER hair removal, or else can remove by regular
clipping. Shaving is not adviced.
Daily Chlorhexidine body wash 7 days before surgery and once a week after surgery for 3
months
Avoid sitting for long hours
If Diabetic this needs to be well controlled
